End of Life Care

Our End of Life Care service provides compassionate, dignified support for individuals approaching the final stages of life, as well as their families.
We aim to ensure comfort, peace, and respect — focusing on what truly matters to the individual and their loved ones.
Our trained carers work closely with healthcare professionals to deliver care that is person-centred, sensitive, and reassuring.

What’s Included:

1. Medication and Pain Management Support

  • Carers administer prescribed medication, including pain relief, as directed by GPs or district nurses.
  • Monitor and report symptoms such as pain, breathing changes, or anxiety to healthcare professionals promptly.
  • Work in partnership with nurses, hospices, and GPs to ensure comfort and effective symptom control at all times.

2. Emotional and Psychological Support

  • We recognise that end-of-life care is not only physical — emotional wellbeing is equally vital.
  • Carers offer empathy, companionship, and reassurance to both the individual and their family.
  • Provide emotional comfort through conversation, listening, reading, music, or quiet companionship.
  • Support loved ones who may be struggling emotionally during this difficult time.

3. Support for Families

  • Provide respite care so family members can rest or take time for themselves.
  • Offer guidance and signposting about what to expect as health needs change.
  • Coordinate communication with healthcare teams, including district nurses, GPs, and hospices.
  • Offer bereavement support or referrals to counselling and community services after passing.

4. Coordination with Healthcare Professionals

  • Work collaboratively with district nurses, palliative care teams, GPs, and hospices.
  • Follow the person’s Advance Care Plan or help create an End of Life Care Plan with them and their family.
  • Ensure that the individual’s wishes and choices — such as preferred place of care or death — are respected and upheld.

5. Respect, Dignity, and Choice

  • Provide care that honours each person’s spiritual, cultural, and religious beliefs.
  • Deliver person-centred care focused on comfort, privacy, and individual preferences.
  • Promote peace, dignity, and compassion throughout every stage of care.
